Living with heart failure - or returning home after you have been discharged from the hospital - can feel overwhelming.

Welcome Home Heart Failure is a pee support programme designed to connect you with trained volunteers who truly understand what you are going through - because they have been there too.

What the Programme offers:
  • One-to-one conversations with trained peers with lived experience

  • Signposting to trusted information

  • Emotional reassurance and understanding

  • Support that fits alongside your NHS/HSC care

Who the programme is for:
  • People living with heart failure

  • Family members, carers and loved ones

What peer support means:

​Peer support is not medical advice. It is about listening, understanding, and reassurance - complementing the care provided by your healthcare team.

Our volunteers are trained to:

  • Listen without judgement

  • Respect your pace, needs, and boundaries

  • Share lived experience, where helpful

  • Support - not advise or replace medical care

You do not have to go through this alone. Whether you are newly diagnosed or further along your journey, we are here to listen.
